Sadly, one person dies from oral cancer every hour of every day in the United States. Close to 40,000 Americans will be diagnosed with oral or pharyngeal cancer this year. Of those newly diagnosed individuals, only 57% will be alive in five years. … We need to observe it earlier! Dentists and ental hygienists are the only health practitioners that observe the oral cavity on a routine basis. We have the responsibility to observe and question oral lesions. However, a recent CR survey reported that only 50% of dental hygienists are conducting potentially lifesaving oral cancer screenings. We can do better! … Fluorescence is a viable adjunct to mandatory visual and manual oral cancer screening because it reveals abnormal cells before they may be noticed visually. … Cancer screening must be a routine part of oral examiniations, and dental hygiensts and dentists should inform their patients of this valuable service they provide.

The most important function of dental care is a thorough assessment and evaluation of the patient’s oral and related systemic health. An exam should never be focused on just one condition, and that includes cancer. One of the biggest mistakes that clinicians make is understating the true value of an oral screening for a multitude of conditions and over-hyping a single disease. The improper communication of these procedures has greatly reduced the perception of the significance and importance of the examination process.
The DentLight Oral Exam Kit (DentLight, Inc., Richardson, Texas) uses a battery handheld light source to stimulate a fluorescence response that is emitted from the tissue. The clinician views the tissue that is being stimulated with this specialized light through specific filters that are attached to the magnification optics of their loupes. If an area of concern is detected, the area can be photographed with a standard handheld digital camera with the supplied filter that can be attached to a lens filter adapter.

A field-of-view screening technology’s initial role is to assess a wide region to discover abnormalities or irregularities. Some of these modalities also have the ability to provide site-specific information to the clinician once an area has been detected. An example of this type of device is a fluorescence mucosal screening technology, such as the VELscope Vx (LED Dental, Inc, www.velscope.com) and DentLight DOE (DentLight, Inc, www.dentlight.com). We’ve put the device through its paces in my office for about 6 months now and have been very pleased with its performance and with the increased visibility it provides. The most important function of dental care is a thorough assessment and evaluation of the patient's oral and related health status. This includes dental caries that lead to cavity, decays under old composite, soft tissue problems such as cheek bite and worst of all, early signs of oral cancer.

Fluorescence from jellyfish is easy to see due to its high quantum efficiency of close to 80% meaning 80% of the excitation light could be converted to the fluorescence emission. However, fluorescence from human soft tissue is much more difficult to observe due to not only lower quantum yield but also broader fluorescence emission.

I’m a big believer in oral cancer screening. By examining and finding cancer at its earliest stages, it allows us to give the patient the best chance to beat this disease. Early on in the development cycle of the Identafi device (way before it was part of DentalEZ) I was tinkering with the device. Cancer screening is something that every doctor and hygienist should be doing. Now with the market seeing multiple devices (Dentlight DOE, LED Dental Velscope Vx) and the Identafi, doctors have more choices in reliable devices than at any time in history.
