Oral Cancer Screening and Head and Neck Inspection are Our Responsibility!
October 1, 2012
Sadly, one person dies from oral cancer every hour of every day in the United States. Close to 40,000 Americans will be diagnosed with oral or pharyngeal cancer this year. Of those newly diagnosed individuals, only 57% will be alive in five years. … We need to observe it earlier! Dentists and ental hygienists are the only health practitioners that observe the oral cavity on a routine basis. We have the responsibility to observe and question oral lesions. However, a recent CR survey reported that only 50% of dental hygienists are conducting potentially lifesaving oral cancer screenings. We can do better! … Fluorescence is a viable adjunct to mandatory visual and manual oral cancer screening because it reveals abnormal cells before they may be noticed visually. … Cancer screening must be a routine part of oral examiniations, and dental hygiensts and dentists should inform their patients of this valuable service they provide.

Never Do Just an “Oral Cancer” Exam!
By Dr. Scott Benjamin
June 22, 2011
The most important function of dental care is a thorough assessment and evaluation of the patient’s oral and related systemic health. An exam should never be focused on just one condition, and that includes cancer. One of the biggest mistakes that clinicians make is understating the true value of an oral screening for a multitude of conditions and over-hyping a single disease. The improper communication of these procedures has greatly reduced the perception of the significance and importance of the examination process.
The DentLight Oral Exam Kit (DentLight, Inc., Richardson, Texas) uses a battery handheld light source to stimulate a fluorescence response that is emitted from the tissue. The clinician views the tissue that is being stimulated with this specialized light through specific filters that are attached to the magnification optics of their loupes. If an area of concern is detected, the area can be photographed with a standard handheld digital camera with the supplied filter that can be attached to a lens filter adapter.
DOE Oral Exam System

The scientific training of the dental practitioner is focused on diagnostic skill. While the clinical procedures that we perform are highly technical and often demanding, it is the detection of disease and the decision-making that precedes treatment that is truly the hallmark of the profession. Thus, the techniques and devices that assist us in becoming better diagnosticians are always welcome. DentLight has introduced the DOE Oral Exam System that helps dentists and hygienists to identify abnormal tissues that may otherwise escape detection. The dental team is tasked with performing regular cancer screenings for patients, and the DOE makes the potentially life-saving examination pleasant and fast. The small, cordless DOE utilizes high-powered LED illumination with a contrast-switching filter that enhances routine visualization. It looks and feels like a portable curing light, only much more versatile. The unit’s easily interchangeable heads allow for a multiple wavelength intraoral examination with violet and white lights that provide an excellent opportunity for the early and clear differentiation of oral tissues. DOE’s unique collimation provides a uniform beam for superior illumination and excitation of fluorescence. The DOE is compatible with digital cameras to allow a permanent visual record that can assist in monitoring oral changes over the short and long term. The smart, nonmemory, rechargeable battery maintains constant power and has enough charge for 20 patients or more. The DOE uses convenient, integrated loupe filters that can be customized for all major magnification brands. The DOE is a modular light source; it converts instantly to transillumination and curing (with an upgrade) with just a simple head replacement. The DOE is all about early detection and prevention.

Advanced diagnostic technologies are the foundation of oral health.
By Scott D. Benjamin, DDS
A field-of-view screening technology’s initial role is to assess a wide region to discover abnormalities or irregularities. Some of these modalities also have the ability to provide site-specific information to the clinician once an area has been detected. An example of this type of device is a fluorescence mucosal screening technology, such as the VELscope Vx (LED Dental, Inc, www.velscope.com) and DentLight DOE (DentLight, Inc, www.dentlight.com). These instruments are initially used to perform a cursory fluorescence screening after the clinician’s visual and tactile examination. This screening is performed to look for any abnormal fluorescence patterns or responses, such as an unexpected loss of fluorescence.
Filtering the Light
Filtering the Light

We’ve put the device through its paces in my office for about 6 months now and have been very pleased with its performance and with the increased visibility it provides. While wearing the magnification and using the filters, tissue exposed to the violet light appears as a greenish color while potential areas of concern show up dark. The contrast is extremely good, which makes the exam process easier to perform. … Patient acceptance has been outstanding. I’ve always done oral cancer screenings and have always made a point to tell my patients about it and why I’m doing it. Now they know that I’m just doing more to try and ensure their health
The most important function of dental care is a thorough assessment and evaluation of the patient's oral and related health status. This includes dental caries that lead to cavity, decays under old composite, soft tissue problems such as cheek bite and worst of all, early signs of oral cancer.
DOE is a portable non-invasive dental oral exam system that addresses all of the above and more. It offers illumination to aid visualization during oral procedures and works as an adjunct to magnify and enhance the visualization of mucosal abnormalities, lesions, dental caries, fractures and endodontic canal orifice without the need of oral rinsing or switching off the room light.
DOE offers a win-win solution for both dentists and their patients. It helps patients avoid health problems that could be cost-prohibitive and even life-threatening at a later stage. It helps dentists, together with other modalities, to market their practice to deliver cost-effective yet profitable procedures to give their patients a piece of mind.
The experience with most doctors when they try their first pair of loupes is WOW!
Loupes provide distortion free magnification between 2x and 8x that improves work quality from a millimeter to easily 50 microns. As a result, any procedures carried with a naked eye will have automatic improvement in the quality and speed of work including margins, contacts, canals, and implants etc.
Additionally, loupes insure you to work at an optimum posture without the need of bending your back to get closer look. Long-term benefits have been proven tremendous in avoiding chronicle back pains and extending career lifetime.
When combined with fluorescence imaging in DOE, loupes ensure consistent soft and hard tissue imaging with less false positives.
Loupes make your work much more enjoyable every day.
Similar to the green fluorescence protein (GFP) that exhibits bright green fluorescence when exposed to blue light in jellyfish, numerous publications [Ref.] in the public domain have established the similar fluorescence in human soft tissue when excited under violet-blue wavelength light.
Fluorescence from jellyfish is easy to see due to its high quantum efficiency of close to 80% meaning 80% of the excitation light could be converted to the fluorescence emission. However, fluorescence from human soft tissue is much more difficult to observe due to not only lower quantum yield but also broader fluorescence emission.
DentLight is one of a few companies that achieved such feat in its capability to image soft tissue fluorescence with high resolution and high contrast. The patent-pending DOE design implements the most advanced technology in LED optics for long-range illumination to the back of the throat and high contrast fluorescence optical filters to maximize emission image contrast while suppressing both room and excitation light.Together with its award-winning modular design, DOE delivers to our customers the portable and reliable solutions for complete oral exam.

When you want to count on quality, functionality and performance, DentLight has proven over and over that we deliver the best products on the market.
DOE is comparable in working principles to Velscope and Identifi, which all depends on fluorescence imaging as the primary detection mechanism. Yet –
- DOE is much smaller and portable than Velscope or Identifi enabling detection of back throat imaging which is a common place of problem.
- DOE has proven integrated rechargeable battery and smart electronics for constant ready exams without the need of changing battery (Identifi) or sleeping mode (Velscope).
- DOE delivers much higher power LED illumination than Identif similar to Velscope yet integrates passive cooling without the annoying fan noise and reliability headache in the long run.
- DOE implements patent-pending high contrast optical filter with much better extinction ratio than Identifi yet integrates with magnification loupes for both consistent high resolution imaging and suppression of room light while other brands need to turn off the ambient light.
- Best of all, DOE is modular and integrates components that can perform a lot more procedures than the other brands including in addition to soft tissue exam
- Composite viewing for old composite removal
- High-power and guided transillumination for dental caries, fractures, and root canal
- Intra-oral Illumination
- Magnification
- Optional fast composite curing
- And what more, the price is right!
Yes. With FUSION, you have the options of upgrading to full DOE or starter DOE, while the latter enables you to perform soft tissue oral exam and composite removal procedures at more than half the price of the competitors.
DOE is a full oral health exam system not just aimed at screening oral cancer. One of the biggest mistakes that dentists make is over-hyping a single disease like oral cancer while not understating the true value of an oral screening for a multitude of health conditions. The improper communication of these procedures has greatly reduced the perception of the significance and importance of the oral exam.
DOE is to discover any abnormality, no matter what it might be, from dental caries that lead to cavity, decays under old composite, soft tissue problems such as cheek bite and worst of all, early signs of oral cancer.
No single device, such as DOE or X-ray machine for that matter, can deliver 100 percent accuracy or effectiveness in detecting conditions such as oral cancer. Detection and diagnosis is accomplished with the accumulation of data from multiple sources and modalities. DOE is an effective screening device for first level oral exams beyond the tip of the iceberg. Signals of problem, e.g., dark tissue with loss of fluorescence, should lead to further diagnosis including salivary diagnosis and oral pathology exams.
The DOE full kit comes with a White Light Head for oral illumination and transillumination. FUSION curing light can optionally add-on White Light Head. It accomplishes two goals:
- Illumination to have side-by-side visual under fluorescence and enhanced white light illumination with the same magnification image
- Transillumination for hard tissue oral health exam including dental caries, fractures, and root canal.
The Exam Tip (also doubles as a Tacking Tip for veneers/crowns/brackets with curing light head) is a disposable black 3mm opening tip that adapts and snaps into the end of the light head to cut down side light emission and guide the light propagation into a focused filament of light. It enables transillumination into interproximal teeth and improves contrast of visual exams on anterior, posterior and endo root canal.
DOE SE is a simplified version of DOE full kit that primarily addresses oral cancer screening with a simple viewing and control system. DOE SE wand operates continuously for 1 minute with a single push of run button. Fluorescence image is viewed through the custom Fluorescence Viewer similar to a standard face goggle. Due to the small size of the LED light and high optical illumination, DOE SE enables both intra and extra oral access which are critically important for the detecton of anterior and posterior regions of the oral cavity.

I’m a big believer in oral cancer screening. By examining and finding cancer at its earliest stages, it allows us to give the patient the best chance to beat this disease. Early on in the development cycle of the Identafi device (way before it was part of DentalEZ) I was tinkering with the device. Cancer screening is something that every doctor and hygienist should be doing. Now with the market seeing multiple devices (Dentlight DOE, LED Dental Velscope Vx) and the Identafi, doctors have more choices in reliable devices than at any time in history.

Fusion light system incorporates: white light – curing light – oral cancer detection light. It is truly the most versatile on the market… The transillumibation differentiates primary and secondary dentin, indispensable as you explore deeper into the root, hopefully avoiding the disappointment of a root ” perforation” and locating the pulp canal to gain access to the canal terminus at the apex.
